Session Planning, Review, and Extension Guidance

This section provides comprehensive operational and clinical guidance for clinicians delivering care within Mindler pathways. It outlines expected session ranges, key principles for early treatment planning, and requirements for clinical reviews and extensions, ensuring high-quality, evidence-based care tailored to individual patient needs.

Overview

Mindler does not apply fixed session limits. Treatment duration is guided by:

  • NICE guidance

  • NHS Talking Therapies standards

  • The patient’s clinical needs, engagement, and response

  • Ongoing clinical review and supervision

Indicative session ranges reflect national guidance and service data and are intended to support proportionate, effective care - not to restrict treatment. Where treatment extends beyond typical ranges, this must be clinically considered, reviewed in supervision, and clearly documented.

Core Principles (All Clinicians)

In the early stages of therapy, clinicians must:

  • Manage expectations (do not promise outcomes or session numbers)

  • Understand the patient’s needs and primary presenting problem

  • Confirm suitability for the selected pathway and modality

  • Identify early where treatment is not effective or appropriate

  • Facilitate timely onward referral where indicated

Our aim is always to achieve the best outcomes with the optimal dose of therapy.

Engagement Between Sessions

Evidence consistently shows improved outcomes when patients engage between sessions.

Clinicians should:

  • Encourage review of transcripts

  • Prompt completion of agreed tasks

  • Use messaging to reinforce goals and learning

Patients who actively engage between sessions often progress more quickly.

Expected Session Ranges (Guidance Only)

PWP – Step 2

  • Assessment: 60 minutes

  • Treatment: typically 3 × 45-minute sessions

Used where presentations are mild to moderate, clearly defined, and patients are motivated to engage. Progress should be reviewed early, with stepping up or extension discussed in supervision where indicated.

CBT Clinicians – Step 3 / Step 3+

CBT delivery at Mindler aligns with NICE guidance and NHS Talking Therapies standards.

  • Assessment: 60 minutes

  • Treatment:

    • Step 3: typically around 6 sessions

    • Step 3+: typically up to 7 sessions

These ranges reflect common NHS practice. Treatment beyond these ranges should be:

  • Based on clinical need

  • Reviewed in supervision

  • Clearly documented

Clinicians must not describe therapy as a fixed number of “sessions” or “hours”.

Counselling for Depression (CfD) – Step 3 / Step 3+

CfD delivery aligns with NICE guidance and NHS Talking Therapies recommendations.

  • Assessment: 60 minutes

  • Treatment: typically 6–10 sessions, depending on:

    • Severity and chronicity of depression

    • Engagement and therapeutic process

    • Progress toward agreed goals

Extensions should be discussed in supervision and remain consistent with CfD principles.

Long-Term Conditions (LTC) Pathway – Step 3

The LTC pathway follows NHS Talking Therapies guidance for adapting CBT to long-term physical health conditions.

  • Assessment: 60 minutes

  • Treatment: typically 6–8 sessions

Treatment focuses on adjustment, coping, symptom burden, and functional impact.
Any extension should be reviewed clinically and agreed in supervision.

EMDR Clinicians – Step 3 / 3+

EMDR at Mindler aligns with NICE PTSD guidance and is offered only for PTSD (Type 1) presentations.

  • Preparation / assessment: 1–2 sessions (as clinically indicated)

  • Trauma processing: typically 6–12 sessions, depending on:

    • Single vs multiple discrete traumatic events

    • Readiness and response to processing

Treatment is phase-based, not session-limited, and progress should be reviewed regularly in supervision.

Mindler does not treat Complex PTSD (Type 2) and therefore does not provide EMDR for this cohort.

Reviews and Extensions (All Pathways)

All clinicians must review progress by Session 3.

Extensions:

  • Are clinically supported and encouraged where appropriate

  • Must be discussed in supervision

  • Require a clear, documented clinical rationale

  • Are required for all clients where a clinical need is identified for further sessions beyond the typical treatment lengths stated above

NICE and NHS guidance emphasise responsiveness to early treatment response, not rigid session limits.

Patients Under Caseness

Patients scoring the below are considered under caseness:

  • PHQ-9 ≤ 9

  • GAD-7 ≤ 7

Guidance:

  • Complete an initial assessment

  • Discuss options with your Supervisor

  • Typically offer up to two psychoeducation-focused sessions

  • Do not exceed this without supervisory agreement

These patients are flagged on IAPTUS as “Non-Caseness.”
